Heavy vehicle operator suffers fatal fall

A truck driver plunged to his death after his employer allegedly failed to take safety measures.

Authorities recently investigated a heavy vehicle fatality at an undisclosed mine site.

The operator lost balance while standing on top of a large container attached to the trailer on 5 August 2023.

“After opening the container lid the driver slipped while descending the container. The driver died from his injuries on August 7,” the Mine Safety and Health Administration said in a safety alert.

Investigators made the following recommendations:

  • install fall protection or safe access to containers or tankers if there is a danger of falling
  • wear proper slip resistant footwear and use three points of contact on elevated surfaces
  • provide site-specific hazard awareness training.
