Heavy vehicle continues driving after brake fails

A mine truck kept moving despite already having an alternate stopping system.

Authorities recently examined a heavy vehicle brake failure at an undisclosed operation.

“The service brake system failed on an unloaded haul truck. The brake failure caused the truck to travel on engine braking for about 100 metres before it stopped near to a workshop area,” the New South Wales Resources Regulator said in an incident summary.

Investigators later discovered the truck had just returned from broken wheel stud repair work, and only operated for about 10 to 15 minutes. The driver did not use the secondary stopping device.

“It appears the truck operator did not apply the emergency brake system,” they said.

The following recommendations were made:

  • implement maintenance systems to maintain functionality of braking systems (service, emergency, park)
  • train truck operators in competently using brake systems, including in emergency situations.
