Heavy equipment crushes resources employee to death

Mineral processing machinery killed a worker.

Authorities recently examined how an employee became fatally injured at an undisclosed operation.

Investigators revealed the crew member had tried to relocate the wash plant when the tragedy unfolded on 24 August 2023.

“In the process of lowering the feed box into the travel position the miner was pinned between the feed box and the hand rail,” the Mine Safety and Health Administration said in a safety alert.

They made the following recommendations:

  • do not work under suspended loads
  • ensure all safety components are in place and working
  • when conducting a non-routine task review safe procedures before starting work
  • block machinery components against motion before beginning maintenance or repairs.
